We help with health, wellbeing, money and accessing services. We can support anyone over 18 who lives in East Suffolk.
Health and wellbeing
The team operate a supported access model - a bridge into services and support - with the aim of increasing confidence, feelings of self-worth and motivation, improving communication skills and increasing trust in services.
We work with people to enable them and help them formulate a plan based around what the person feels would improve their life, including:
- improved mental and physical health
- improved family relationships
- increased trust in services
- help to secure appropriate housing
- help to secure employment, education or training
- reduced social isolation and loneliness
The team will not work with individuals who are already being supported by a named practitioner, e.g. adult social care or mental health services.
