Community Help Hub

Our Community Help Hub Team provide front line support to some of our most vulnerable residents.

The Community Help Hub (Adults and Families) team works to support everyone who lives in East Suffolk, with a particular focus on deprived, marginalised and excluded communities.

The team operate a supported access model – a bridge into services and support - with the aim of increasing confidence, feelings of self-worth and motivation, improving communication skills and increasing trust in services. 

The team work with people to enable them and help them formulate a plan based around what the person feels would improve their life, including:

  • improved mental and physical health
  • improved family relationships
  • increased trust in services
  • help to secure appropriate housing
  • help to secure employment, education or training
  • reduced social isolation and loneliness

The team will not work with individuals who are already being supported by a named practitioner, e.g. adult social care or mental health services.
